How do I sign a declaration with a family doctor/therapist paediatrician?

To sign the declaration, you need to:

1. choose a medical institution and a doctor with whom you want to sign the declaration;

2. call the selected medical institution and find out the details of the visit to sign the declaration;

3. prepare the documents:

- an identity document (passport, refugee certificate, etc.);

- a certificate of registration number of the taxpayer's registration card (tax ID); and

- a child's birth certificate (if you need to sign the declaration for a child under 14 years old);

4. contact the registration office of the medical institution and inform them that you want to sign the declaration.

The employee of the medical institution will enter your data into the electronic system. You will receive an SMS with a code on your mobile phone. You need to tell this code to the employee of the institution; and

5. sign the declaration filled in based on your data.
