What documentation and procedure are needed to obtain PiH status under the typical mechanism?

Special commissions at each ministry or security body review documentary evidence such as official participation certificates (forms 1, 4, 6, 7), extracts from orders (including of the Anti Terrorist Centre), combat reports, logs, testimonies from commanders or witnesses (at least three, including the commander) and medical certificates. A decision is issued within one month; if evidence is insufficient, documents are returned for revision. Disputed cases may be considered by the interagency commission under the Ministry of Veterans Affairs.
