Which documents should be submitted for reservation of conscripts?

To reserve persons liable for military service, the employer submits the following documents:

1. Basic documents:

  • A request for the reservation of persons liable for military service addressed to the relevant line ministry, state authority or local self-government body.
  • A list of employees to be reserved, including:
  • Name of the person liable for military service;
  • Date of birth;
  • Identification number (individual taxpayer identification number);
  • Military occupational specialty (MOS);
  • Number of the military registration document (military ID card or temporary certificate);
  • Position and functional responsibilities.

2. Additional documents depending on the category of the enterprise:

For companies performing mobilisation tasks:

  • Copies of documents confirming the existence of a mobilisation task (contracts, agreements, etc.).

For critical enterprises:

  • A decision of the authorised body recognising the enterprise as critical for the national economy, security or defence.

For enterprises, institutions or organisations that have contracts with government agencies:

  • Copies of contracts or agreements specifying the company's obligations.

3. Documents from employees:

  • Copies of the employees' military registration documents (military ID card, temporary ID, etc.).
  • Copies of passport and identification number.

4. Relevant cover letter:

  • Indication of the grounds for booking in accordance with the applicable law.
  • Justification of the need to book each employee.

Where to submit documents:

  • The documents are sent to the relevant state authority (for example, the Ministry of Economy, the Ministry of Agrarian Policy or another authority depending on the scope of the company's activities).

Note:

  • If the application is approved by the General Staff of the Armed Forces of Ukraine, the decision on the reservation is made by the Ministry of Economy.
  • After approval, the employer prepares an extract from the reservation order and provides it to the employees.
