In order to restore the conclusion of an inclusive resource centre (IRC) on a comprehensive psychological and pedagogical assessment of a child's development in case of loss, parents or other legal representatives of the child can do the following.

Obtain a copy through online services

  1. Use the IRC portal. To do this, you need to register on the platform, after which you can download a copy of the conclusion.
  2. Use the Client-IRC mobile application, which can also be used to download a copy of the conclusion.

Applying to the IRC at the place of temporary residence

  • If you were unable to obtain a copy through online services, you can:
  • Personally contact the nearest IRC with a request to provide a copy of the conclusion.
  • Submit an application for an initial or repeated comprehensive psychological and pedagogical assessment of a child's development.

To apply to the IRC, you must have:

  • An identity document of the parents or legal representatives.
  • The child's birth certificate.
  • Individual rehabilitation programme for a child with a disability (if available).
  • Form 112/0 ("Child Development History") from a medical institution.
  • If necessary, a certificate from a psychiatrist.

Please note:

  • You can apply to the IRC regardless of the original place of assessment.
  • The new conclusion issued after the reassessment will have the same legal effect as the lost original certificate.
