The institution shall be connected to the electronic healthcare system. Otherwise, doctors will only be able to issue paper prescriptions.
You don't need a signed declaration with a family doctor. Any doctor can issue a prescription - it can be a primary care doctor (pediatrician, therapist) or a specialist doctor (dentist, surgeon).
If necessary, the doctor forms an electronic prescription and gives the patient an information certificate about the prescription.
You can buy prescription medicines at any pharmacy by presenting the information certificate.

Pharmacies located in the non-government-controlled areas of Ukraine sell antibiotics without a prescription for the duration of martial law.
