A state notary.
A private notary.
Details:
The permission must include:
Confirmation of one parent's consent for the child to leave the country.
The duration of the child's stay outside the country.
The country or list of countries where the child is allowed to travel.
For notarial registration, the following documents are required:
Parents' passports and tax identification numbers.
The child's birth certificate.
Other documents depending on the situation (e.g., documents confirming the guardian's status).
Note:
The child's departure is only possible if it complies with the Ukrainian legislation, particularly regarding the destination, accompaniment, and purpose of the trip.
