When crossing the border with Poland documents verifying identity are necessary. If a child does not have a passport, a birth certificate will be useful.
─ If the validity of the foreign passport has expired:
If you entered Poland with a biometric foreign passport, the validity of which has already expired or will expire within the next three months, you can contact the nearest Ukrainian Consulate in the queue (one of the five - Warsaw, Krakow, Lublin, Gdańsk, Wrocław) to extend the passport's validity. For this, you need to have the original passport and its copy.
An extension of the passport can be issued no earlier than 2 months before the date of expiry of the passport.
If the period of validity of the foreign passport of a child under the age of 16 is extended, his/her personal presence at the Consulate is not mandatory.
