What to do in case of loss of death registration certificate?

The following methods can be used to obtain a duplicate death certificate:

1. Through the Diia service:

• Log in to the Diia platform using an electronic signature.
• Fill out the form for renewal of the certificate.
• Pay the administrative fee through the service.
• Choose a convenient Ukrposhta branch or civil registry office to receive a duplicate.

2. In person at the registry office:

• Contact the nearest Civil Registry Office.
• Write an application for re-issuance of the death certificate.
• Provide a document proving your identity.
• Pay for the service of issuing a duplicate.

Important: When applying, you must have an identity document and a receipt for payment of the administrative fee.
