What to do in case of loss of death registration certificate?

There are several options for obtaining a duplicate death registration certificate:

1. Using the "Diia" service:

- go to the link and log in using an electronic signature;

- fill out the form for re-obtaining the certificate;

- pay for the service;

- choose Ukrposhta branch or DRATSS branch that is convenient for you to receive a duplicate certificate.

2. Order in person at the DRATS branch:

- find the nearest department of DRATS;

- write an application for reissuance of a death registration certificate;

- pay for the service.
