What to do in case the birth certificate is lost?

If a birth certificate is lost, stolen, damaged, or destroyed, it can be reissued through the Civil Registry Office.

Procedure:

  1. Apply:
  • To the nearest registry office.
  • Or use the Diia portal.
  1. Submit an application:
  • Write an application for re-issuance of the certificate at the Civil Registry Office.
  • Or fill out an online form on the Diia portal in the Services section.
  1. Pay for the service:
  • The cost of the service is currently 51 kopecks.
  1. Get a new certificate:
  • In person at the Civil Registry Office.
  • Or order delivery by mail.

Who can apply:

  • Parents or adoptive parents of the child.
  • Legal guardians.
  • The child, if he or she has a passport.

Important:

  • A reissued certificate has the same legal force as the original.
  • If you are abroad, you can apply at diplomatic missions or consular offices of Ukraine, as well as through the Ministry of Foreign Affairs of Ukraine.
