What to do in case the birth certificate is lost?

If a birth certificate has been lost, stolen, damaged or destroyed, it can be reissued through the Civil Registry Office.

Step-by-step procedure:

1. Applying to the civil registry office :

  • In person at the nearest Civil Registry Office.
  • Through the Diia portal (Services section).

2. Filling out the application:

  • Submitting an application for re-issuance of the certificate at the Civil Registry Office.
  • Filling out an online form on the Diia portal.

3. Payment for the service:

  • The service fee is currently 51 kopecks.

4. Obtaining a new certificate:

  • The certificate can be obtained:
  • In person at the Civil Registry Office.
  • By post, if this was specified when submitting an application through Diia.

Who can apply:

  • One of the child's parents.
  • An adoptive parent or legal guardian.
  • A child, if he or she has a passport (from the age of 14).

Abroad:

  • You can apply through diplomatic missions and consular offices of Ukraine.
  • Alternatively, through the Ministry of Foreign Affairs of Ukraine.

Note: A reissued certificate has the same legal force as the original one.
