You do not need to submit additional documents to make an appointment with your family doctor, with whom you have signed a declaration.
If you are unable to visit your family doctor during martial law, you may visit any primary care physician (therapist, pediatrician) for medical care. It is not necessary to have a declaration with a family doctor.
To do this, you need to have the following documents:
- an identity document (passport, refugee certificate, etc.);
- a certificate of registration number of the taxpayer's registration card (tax ID); and
- a child's birth certificate (if the child needs assistance).
Additionally, you need to provide the following information:
- medicines that the patient takes regularly; and
- surname, name, patronymic of your family doctor.
You can make an appointment:
1. online, if you know whether the medical institution is connected to the medical information system (the "MIS") and to which one. MIS is a special system that allows you to choose a doctor and make an appointment;
2. by contacting the registration office of the relevant medical institution; or
3. by visiting a doctor on duty without an appointment.
