1. If a declaration is signed with a family doctor:

  • No additional documents are required.

2. If you visit another primary care doctor (general practitioner, pediatrician) during martial law:

1. A declaration with a family doctor is not required. For an appointment, you must have

  • an identity document (passport, refugee certificate, etc.)
  • a certificate of the registration number of the taxpayer's account card (identification code);
  • a child's birth certificate (if the appointment is necessary for the child).

2. Additionally during the visit:

  • Provide information about the medicines that the patient takes regularly.
  • Provide the surname, name, patronymic of the family doctor (if any).

Ways to make an appointment:

1. Online:

  • If there is a medical information system (MIS) in the facility that allows making an appointment.

2. In person at the registration office of the healthcare facility.

3. Without an appointment - by contacting the doctor on duty.
