- Grounds for revocation:
- A death record may be canceled if a court determines that the record is false (for example, if the person in respect of whom the record was made is found to be alive).
- Where to apply:
- The civil registry office at the place where the death certificate was drawn up.
- Documents for revocation:
- The applicant must submit:
- Passport of a citizen of Ukraine or a document certifying a stateless person.
- A certificate of state registration of a civil status act, which is subject to cancellation.
- A court decision recognizing the death record as invalid.
- Term of consideration:
- The issue of revocation is considered within three months from the date of submission of the application.
- Result:
- After the decision to cancel the vital record is made, a corresponding certificate is issued to the applicant.
- The data is entered into the State Register of Civil Status Acts.
