1. Seek medical care from a local doctor in the country of residence.

• The doctor must provide documentary evidence of temporary disability (certificate, extract or other medical document).
• The document must contain:
• The patient's full name,
• diagnosis
• duration of treatment,
• signature and seal of the doctor or medical institution.

2. Send the received documents to your doctor in Ukraine:

• the documents must be translated into Ukrainian;
• the translation must be notarised (if necessary).

3. Your doctor in Ukraine, on the basis of the submitted documents:

• assesses your health condition;
• draws up a medical certificate of temporary disability;
• issues an electronic sick leave certificate (E-sick leave).
