Information about the governing bodies of a non-governmental organisation is drawn up in the form of a table containing the following information:

  1. Full name of the person who is a member of the governing body.
  2. Position held by the person (chairman, deputy chairman, board member, etc.).
  3. Date of birth.
  4. Registration data (passport data or information about the identification code).
  5. Address of residence.
  6. Contact details (phone number, email).

Information on governing bodies is signed by the chairman of the constituent assembly and the head of the organisation. Additional certification by all members of the governing body is also possible.
