Due to the war, I cannot contact my family doctor/therapist/paediatrician, can I visit another doctor?

You may contact another primary care physician (pediatrician, therapist) if you have no contact with your family doctor.


You need to provide the following documents to the doctor:

- an identity document (passport, refugee certificate, etc.);

- a certificate of registration number of the taxpayer's registration card (tax ID); and

- a child's birth certificate (if the child needs assistance).


Also, you need to provide the following information:

- medicines that the patient takes regularly; and

- surname, name, patronymic of your family doctor.


You also need the above documents if you want to re-sign the declaration and choose another family doctor.
